Хронический тонзиллит и эпителиально–эндотелиальный дистресс-синдром: клинические и этиопатогенетические параллели

Аннотация

Теория эпителиально-эндотелиального дистресс-синдрома («ЭЭДС-НИКИО») предложена в качестве системного решения актуального вопроса хронического тонзиллита в спектре патологии ЛОР-органов. Положения этой теории позволяют выделить важные этиопатогенетические механизмы развития хронического воспаления в нёбных миндалинах, а также системные воспалительные метаболические параллели с развитием эндотоксинемии и хронической эндотоксиновой агрессии. Последнее определяет системное хроническое воспаление, поддерживающее регионарное воспаление миндалин. Все эти процессы происходят в результате формирования несостоятельности первого рубежа защиты – слизистой оболочки с ассоциированной регионарной лимфоидной тканью. Существенным, если не определяющим, при этом является развитие несбалансированности микробиоценозов ротоглотки и кишечника. Таким образом, исследование и изучение механизмов формирования хронического воспаления в нёбных миндалинах в логике теории «ЭЭДС-НИКИО» с развитием эпителиально–эндотелиальной дисфункции, как регионарной, так и системной, которая по мере истощения протективных сил организма прогрессирует в виде эпителиально-эндотелиального дистресс-синдрома. Данный подход является фундаментальной базой решения проблемы хронического тонзиллита, а также сопутствующих и сопряжённых с ним заболеваний.

Annotation

The definition of epithelial-endothelial distress syndrome (EEDS-NIKIO) is proposed as a systemic solution to the urgent issue of chronic tonsillitis in the spectrum of ENT pathology. The provisions of this theory allow us to identify important etiopathogenetic mechanisms for the development of chronic inflammation in the palatine tonsils, as well as systemic inflammatory metabolic parallels with the development of endotoxinemia and chronic endotoxin aggression. The latter determines systemic chronic inflammation that supports regional inflammation of the tonsils. All these processes occur as a result of the failure of the first line of defense — the mucous membrane with associated regional lymphoid tissue. Significant, if not decisive, is the establishment of an imbalance in the microbiocenoses of the oropharynx and intestines. Thus, the study and investigation of the mechanisms of formation of chronic inflammation in the palatine tonsils in the logic of the theory «EEDS-NIKIO» with the development of epithelial-endothelial dysfunction, both regional and systemic, which, as the body’s protective forces are exhausted, progresses in the form of epithelial-endothelial distress syndrome. This approach is the fundamental basis for solving the problem of chronic tonsillitis, as well as concomitant and associated diseases.
